| Update on Gizmo Thank you everyone for your prayers and kind words. They were really needed. Anyway, the blood results came back for Gizmo's acid bile test. The pre-screening enzymes were low-I think the number was 2- however, the post screen of the enzymes was 22 and I think it needs to be under 15 for be in the normal range. Forgive my lack of knowledge, I'm a little upset while I'm typing this. After hearing the news, I asked the emergency vet to fax over the results to my normal vet and I have an appointment at 3 to have them go over it with me and discuss our options. I guess all I'm asking for is some advice. I know that I'm going to do whatever it take to get Gizmo feeling better, and I don't care about the cost right now I have my options that my husband and I discussed. I just Gizmo to love his life and not be in pain. The emergency vet said they'd recommend him to a specialist to get an ultrasound, go figure it was the specialist that I took my Cairn too when she had cushings. Anyway, I guess I'm asking for anyone who's gone through this what the possible process is. Does the dog have a good quality of life? Am I doing the right thing?

| 22 is a very, very low - high. Most vets would probably pay zero attention to a 22. Dr. Center (develper of bile acids test) has a cut off of 25. His enzymes may have been up because he wasn't feeling well or his liver was just a little irritated about something. Heartworm meds, flea meds, and vaccines can potentially make them go up too. Ellie's bile acids came back at 47 post, then 15 post, then 106 post. When it went to 106, we did go to a specialist (I assume the same one that you have a recommendation for) for an U/S. If your vet wants you to go or you would be more comfortable, then go for it. I really wouldn't expect any liver abnormalities as far as shunt goes with that number though. Maybe MVD? ..which wouldn't be seen on U/S. I think you can be about 99% sure this is not a shunt. Theoretically possible, but highly improbable.

| I would do what my general DVM wanted me to do... If she gave me options, (with a post of 22), I would choose between them. If Gizmo does have MVD, that is important to know because anesthesia and analgesia protocols would be tailored to meet his needs. However, even if you go to a specialist, if the liver looks fine on U/S with a post of 22, they almost certainly won't do the biopsy to confirm anyway (well, maybe they will - but I don't think that's a great idea in most cases). Anyway, I'd be interested to see what the liver enzymes are doing in a few days. Depending on how high they are elevated, you'll need to know if they went back down. Young dogs shouldn't have high liver enzymes. There is usually a reason. It could have been just the liver getting irritated from intestinal issues, but you need to make sure they go down. I know the first r/o around here lately in young dogs with unexplainable liver enzymes is lepto. Doubt he has it, but it's the major thought here now... It's coming up quite a bit. Do what your vet thinks. If I were in this case, no way would I pay for a U/S at a specialty hospital, but my vet would probably agree with that choice. Also if this is where I think it is, they are a very, very, very good specialty hospital and I loved E's IMS, but I didn't feel like they are great with the liver or anything and I didn't really like some things there. If I had to do the diagnostics over again, I'd probably just head to MSU personally. Maybe I should word this another way.. I felt like a good job was done on the U/S, but the conclusion that was reached I could have reached myself. Just wouldn't be excited to take another liver compromised dog there.

| Just another update. My vet looked at his results and wasn't so inclined to do a U/S yet. Her whole thought process is that this all resulted from us gradually switching Gizmo over to an adult food which can cause a big GI upset. She also stated that she's had many dogs come in with GI issues similar to Gizmo's and it was an infection in the tract and antibiotic seemed to do the trick. She thought that 22 was above, but not really high (sound familiar Crystal? ) However, she didn't want to rule it out completely since it' so common in the Yorkie breed. And increase in lypase can simply be from just a GI upset. She watched him physically interact and also stated that his balance isn't off, he's not confused and it's good that he wants to play so badly. In the end, we were sent home with a can of Science Diet W/D because she too thought that L/D was too high in fat. And also said to entice Giz I could warm up some chicken stock and put it on the food as well. We were also sent home with ear drops because he has a yeast infection in his left ear and I actually had an extra mix of Clavamox at home and was told to give him it twice a day. He's going to have a new blood panel done in 10 days to see where everything is at. I can't thank everyone enough for the kind words. Gizmo right now is sound asleep on his bed in the kitchen sunbathing. It was a rough day. Also, when we got home, I mixed in the wet food with the stock on top of the dry, and Gizmo literally went bananas and ate almost everything.

| Glad you got to your vet to find out what is going on with Gizmo. Sounds like she is going to be watching Gizmo carefully. We will hope that the next blood panel comes back with good results! Sending prayers and hugs to Gizmo. From Mikki and Rosie!!!

| Thanks Brit! I know Jackson went through this as well, and I know he's a chewer too. Were you recommended anything that he can chew that isn't too fatty?
