I do not think there is any 'legal form' I am sure that you can just create something saying that she is indeed in right mind surrendering him to ......

I am sure if you asked one of the Rescues, they could give you one or at least ideas

Jeri you're a wonderful person to take this sweet little baby. I had tears when reading about the seizures and almost gasped about the heratworm deal. He's so pretty and you did a great job with the grooming.

I am taking him to meet Mary this weekend. We are camping in Old Sac. and Mary is bringing pizza. Maybe it will be love at first sight??

My niece is already so attached to him but can't have a dog where she is.
I'm trying really hard to not get emotionally involved.
I really can't keep him myself.

When you have her sign, I would just make sure you have it notarized. I work with an adoption agency, and our child relinquishments are very long and wordy. However, the most important thing would be that her name, the dog's registered name, and your name all be included as well as her signature and a notary as well as the date. That way you have proof that is legal. Good luck.

Well Mary ( Grizzley Bear) met Teenie and they didn't click BUT my niece and Teenie are getting very bonded.
I got a very nice card from Teenies owner and she sent 20.00 in it. I think she may want him back so well see.

Teenie really loved camping. He did so well and loved sleeping on the steps while we were outside.
