When you have her sign, I would just make sure you have it notarized. I work with an adoption agency, and our child relinquishments are very long and wordy. However, the most important thing would be that her name, the dog's registered name, and your name all be included as well as her signature and a notary as well as the date. That way you have proof that is legal. Good luck.
