Chloe and Potty Training

My little Yorkie, Chloe, will be 1-year old in March. She weighs 4 lbs and is a ball of fire. Potty Training is still an issue. She has no concept that peeing and pooping both fall into the category of "pottying". She is outdoor trained, mostly. She used to use the potty-paper but won't use it anymore! When I leave her in the house over 3 hours, she does #1 and occasionally #2 throughout the house (4 small rooms) Although she will go 12 hours at night without pottying (she sleeps with me), How long can Yorkies be expected to wait and how do I train her to do that at her age?
All suggestions are welcome. I don't know how to solve this issue.
