Possible MVD or Liver Shunt Hello YT. I have recently joined this board and have read a few of the informative post regarding MVD and LS and would like some insight to my case. My yorkie boy (Peejay) is 16 months old and noticed a head and shoulder wobble late Friday night. I have seen a slight head wobble a few months back but didn't pay much attention to it since it went away quickly. This time, it was much more severe and he seemed disoriented with trouble walking during this episode. I brought him to the 24 hr clinic and the ER vet explained the possibility of LS. The blood work was done with ALT 29, ALB 3.4, BUN 34, and rest normal besides GLOB 2.4, HCT 56.5%, and 19.4. The urine test results indicated a urinary tract infection. The bile acid test was done Monday and the results for pre 34 and post 58. Our usual vet is requesting a ultrasound done with a specialist. I have read from other posts that this is only paritally accurate and a Protein C test should be done instead. I plan to start him on a L/D diet asap. What do you guys recommend as the next step? Should I consult with a third vet? Thanks in advance. |

Hi, I'm sorry your baby is having some problems. Those results don't seem completely conclusive of LS. Ultrasound would not be my next step at this point. I would probably just change the diet to L/D canned or preferably Royal Canin Hepatic LS 14 and put him on Denosyl and/or milk thistle. Then, I would retest in 30 days and make sure he doesn't have any heartworm or flea meds within 2 weeks of the new BAT. If the results are still even a little off, you know you're dealing with an issue in the liver. OR you could just go ahead and ask for a Protein C test which will help determine if your baby has a shunt or if it's more likely to be MVD... Try to feed small frequent meals instead of 1 or 2 regular meals. It's best to feed every few hours SMALL meals to help lighten the load on the liver. Did they check for crystals in the urine?? This is very important and many times, vets just skip telling you that part. Ask for a copy of the urinalysis results, and read it yourself. Look for anything that might say ammonium urate or ammonium biurate. |
I would skip the ultrasound. According to Dr. Center, they can be as low as 60% accurate. Save your money. Since the cut off is less than 25, his bile acids are abnormal but not terribly high. Bile acids over 100 are usually indicative of a shunt, but head wobbling and disorientation are classic signs of a shunt. I would go ahead and get a Protein C test done. I have made another appointment to consult with a specialist that will decide if an ultrasound will be needed. I plan to skip the ultrasound and will request a protein C test and ask for a copy of the urinalysis test from my usual vet. He is now on a L/D diet and has been prescribed antibodies for the infection, lactulose, and denamarin. Thanks again. |
