I am trying to design a Puppy Application Form.

I don't want to forget any important point so I was wondering if you breeders could show me the form that you are using. It would be very helpfully to me and extremely appreciate.

You are very welcome! Feel free to use it and tweak it to what your comfortable asking, and add whatever I might have left out. I just use that for step one. After I get the answers back, If they passed my expectations, I conduct an interview at their house will all the family members.

I want to use it mostly to compare the potential families and help me decide which one I will allowed to visit the puppies first. I have talk and e-mail to so many people regarding this litter, this is getting really confusing :questione

Thanks for posting this question Chantal -- I too, am working on mine. Would appreciate any suggestions. I don't want to forget anything, but I don't want to ask things that might not really matter either.

By applying to receive a puppy from us, you are subject to a home visit and interview by the breeder in order to verify that your home is suitable for a Yorkie puppy. We offer a genetic health guarantee for all our puppies up to the age of 4 months.
You will be able to pick up your puppy after they have reached 8 weeks. The puppy will have received their first vaccinations, deworming and a health check. You will receive a free puppy pack with loads of goodies to get you and your new best friend started. I also include an information booklet that is geared towards caring properly for you new Yorkshire Terrier.

We hope to be part of your family's life for the duration of the puppy's lifetime. We are always open to questions - no matter how silly. We would also love to see photos of your puppy as it grows. We also aim to educate new Yorkie owners so as to ensure that your puppy will have a long and healthy life.
