Ok, first, I am NOT a vet...I am new to breeding and have only had two litters...now, my honest opinion, and my gut feeling is, that is the puppy's intestines and they developed outside of her body.
IT IS VERY POSSIBLE THAT I AM WRONG. That is just MY conclusion from the picture you posted. You need to call your vet ASAP. Please.
