I'm so sorry you have to endure something like this!! It is hard to believe that this girl is so worried about Maddie being near Lily when she isn't bathed regularly and she isn't properly sterilizing bottles and nipples.
I think I'd go along with V on this one and sit her butt down and tell it like it is. It doesn't sound like you're going to be in a winning situation here, so you might as well lay it on the line and hope for the best.
