This is what I would do...
First of all you should not offer a health guarantee, it would be pointless since you don't know the lineage and you would be taking a HUGE risk by doing this. The only health guarantee you could possibly offer is that your puppies will be healthy when they get them.

Questions to ask are....
Have you ever owned a yorkie or a toy breed or are you familiar with the breed (toys need special care being that they are so small and can get hurt real easily)

Have you ever owned a dog?

List your current animals you have

If they have owned a dog or yorkie ask for contact information for their vet, this way you can see how past or current dogs are being taken care of

How many hours a day will the puppy be home by himself/herself, you don't want to sell a puppy to someone who will be home only for a couple hours

Are you familiar with hypoglycemia??

If your pup needs training, are you willing to do the training yourself or enroll the puppy in classes?

Do you have young children and would they be able to properly handle a yorkie?

Do you take full responsibilty for the puppies overall wellbeing and caare and are you able to provide your puppy with routine vet care and flea and heart worm prevenatative?

Are you willing to provide grooming care to your puppy, alot of people don't realize that their coats need alot of attention

Where will your yorkie puppy be housed?

Do you realize that yorkies are strictly indoor dogs and should never ever be housed outdoors??

What personality trait best fits your life style, active (best for working owners, traveling, or children) or quiet (best for lap lovers, stay at home owners, only adults in household, generally living a quiet life)

Also if you write up a contract, you can put in there that the puppy must be spayed/neutered by a certain date, 6,7 months, what ever you decide. You can also offfer that if they do get them spayed/neutered by that date you will give them some money back, and will release the registration papers to them.

Anyways these are some of the questions I came up with, I am sure there are many more. Good luck with your babies!!
