That happened with one of my cats... They used internal sutures that were supposed to dissolve. The knot didn't dissolve all the way though and her body eventually pushed it to the surface.

It wasn't that big of a spot though and the fact that it looks like there's junk in it would concern me (but that could just be the stitch you're seeing). If it seems to get any worse I'd probably call again and see if they'd want to check it out. Hopefully they wouldn't charge for an office visit 'cuz it's more or less follow up...
