I would wait until he is about 8 to 10 mos. to see which are going to fall out on their own and then have him neutered at the same time. The teeth on the outside are the baby teeth. We all worry about anesthesia, but they are even spaying 8 week old puppies now. Just make sure you have the preop blood work done. It's worth every penny and will mostly likely show if they are going to encounter any problems. Make sure your vet is experienced with toy breeds. Make sure they use isoflurane and do not give a barbituate preanesthesia injection. Hopefully, he'll gain to at least 2 pounds and the bloodwork doesn't show any liver problems.
