Me, me me, I use a bell.
I did NOT crate train, but did use a pen just as you are. Whenever I took them out of the pen I would walk them to the door, set them down and put their paw on the bell to make it ring. Then open the door and let them out. Lexi and Tori were both scared of the bell the first few times but caught on quickly. Candi was never scared of it and it seems to be a game to her, but everytime she rings it I open the door.
