I think the heartworm pill is less risky than getting heartworms. I've not heard of anyone having problems with them. I'm gonna start Lexy on them next week. I've just wormed her and didn't want to put too much in her little system at once, but I think she will handle it fine. She is 1 3/4 lbs. at 4 months old and the vet didn't hesitate to give her heartworm pills.
