Default Homemade Yorkie food - advice needed

Hello,
I have a very active 1 year old yorkie name Sophie. She is 4 lb now. She has been doing very well on Royal Canine Puppy food but when I switched her to Royal Canine Small breed adult food when she turned 11 month old, she started to have fever, bloody diarrhea and her health deteriorated very quickly. After vet visits, she was fine then it started again and it became a cycle. I lost my last yorkie girl and she had a condition but this reminded me of her so much. This time, I kept thinking that it's the food that is causing this problem.

About a month ago, when this happened again, I stopped giving her the dry food completely. I stopped giving food for 24 hours and gave her Pepto and it stopped the diarrhea. I cooked white short grain rice and added warm water and gave her for a day. And her stool was better. Then I started to add natrual food like sweet potato and carrots and freeze dried chicken/ beef liver(Crump's natrual brand). And ever since then, this did not occur anymore. I went to local fish market and found a pack of Salmon skin that was packaged after they fillet the salmon. It was like 3 bucks for 3 full salmon skins & tiny bit of fillet meat from bone. I rubbed them with olive oil and baked them 350 degrees for about 40 minutes and made a huge bags of salmon treats. I also gave her unsweetened greek yogurt but noticed that her stool became littlebit liquidy so, I stopped.

Anyhow, I am not sure by doing all this myself, I am taking away any nutrients and need to add vitamins or supplements to the diet.

Any suggestions on whether I can continue with these food?
