Do you ever have a professional cleaner come out?
Even though I clean periodically on my own, have a spot bot, I feel it's nothing like a professional deep clean with protectant and deodorizer.
That may help with the carpet smell issue.
As for the accidents going in the same spot, I'm sorry I don't have a solution as it seems like you're trying everything.
