Just a thought and I apologize if it's already been said (I read lots of the posts but there are tons), you need to think like a dog. "Mom gets mad when I pee so I better go pee away from her". The dog doesn't realize you are mad about the location. He had just learned that peeing makes you mad so why would he want to pee on a pad right in front of you? Much safer to run back to the living room and pee really fast before you see him. Another thing about the rubbing his nose in it thing, dogs don't think pee is gross. They actually think its pretty cool so rubbing his nose in it isn't any kind of punishment to him anyway.
