Yes, it is indeed a bigger needle. Bella probably could have handled it. She's absolutely a perfect lady for the people in the vet's office, and a screaming banshee at home. When we took Bella to the vet yesterday to have her eye looked at, and she held perfectly still while they stained her eye and put in eye ointment, the vet said that Bella deserved the "good dog of the day" award. I told them that Bella is nothing like that at home. Indeed, when it came time for us to put the ointment in her eye, she was squirming and keeping her eyes closed the whole time. It took a few attempts to get the ointment in her eyes.

But back to the subject at hand, I agree that it's best to have the chip inserted when the dog is under anesthesia for another procedure. It's not necessary to be under anesthesia for the chip, and dogs are very forgiving (according to our vet), but since we had the option, that's what we did. It probably worked out better for her and spared her some pain. If I did have the chip inserted without anesthesia, I would definitely want to be present to see how much pain is actually involved.
