Poor Alfie and poor you! You both deserve a break from worrying.
The coughing and gagging could be a throat irritation. If it started immediately after surgery, probably from intubation.
Could also be the licking and hair, as you suggested.
If it is really bad, then he could have an inflamed throat. Max had an episode once that made me think he was choking. The coughing and gagging were really bad. We rushed him to the ER and it turned out his throat was inflamed. Antibiotics fixed him.
