All heartworm medications--chewables or topicals--go into the bloodstream. It's a matter of personal choice, and what gives the dog fewest side effects. Some dogs tolerate some heartworm medications better than others. Bella does great on Revolution (a topical). The reason she started on Revolution is because they make a dose for dogs under five pounds. Since she does well on it, we never had a reason to change. (BTW, Revolution does leave a small amount of residue on the skin and fur, kind of like dried glue, but it's not a big deal.)
