Maybe he keeps slipping and slightly re-injuring it, one of those repetitive motion type injuries, and fails to put full pressure on his feet, fearing re-injury and so he keeps slipping. Can you try a trail of secured area rugs to see if that gives him some stability and allows his wrist or foot or whatever it is to heal? A couple of months of area rugs where he slips the most and that cover the areas he frequents most could be worth it to see if he eventually stops holding his foot up - which kinda leads me to thinking he's got some tenderness or pain. What do you think?
