The need for the cone depends on if you think he will bother his stitches. I strongly suspect the cone is throwing him off his good potty habits.

I agree with Gemy, basically you will have to go back to potty training 101, supervising, limiting roaming freedom, putting him on the pad to remind him that it is the correct place. Always praise when he gets it right and don't focus on scolding when he has an accident. Just say "uh oh, supposed to do that on the pad" and bring him to the pad.

My boys are 5 and 6 and we still praise for good potties like they are puppies in training.
