I should let someone from NY answer this, but in my province, you get a rabies tag from the veterinarian when you do the rabies, then when you license your dog you get a license tag. Here we can pay a bit extra and have our contact info placed right on the license tag. You are supposed to put both tags on their collars - but don't use the collar to attach the lead to, for Yorkies halters are best.
