It looks like a staph infection, in which case he'll need a course of antibiotics, or possibly a yeast infection, in which case he'll need a fungicide.
Either way, he is uncomfortable and suffering. Take him to the vet! If your vet is out of town, take him to another vet. For initial relief of his discomfort, this should not be an expensive vet visit.
