Did they do an oblique cervical X-ray? I don't think I would be going the internal med route. I would be going to orthopedist or neurologist for this, not internal med. I could be wrong but this sounds mechanical to me. I'm not a vet, but that would be where I would go if this were my dog. I don't think this sounds normal.
