I've got nothing good to say here, I'm sorry for the maltese...she needs immediate care, a 4lb. maltese breeding with an 8lb. anything stacks the odds against her health. Not sure why your aunt would have ANY intact dog around her intact dog...not sure why these dogs aren't spayed/neutered, have no idea what is going on here at all, all I know is your aunt needs to get this pup to either a vet for an emergency spay or to a repro specialist...this is a serious situation.
