Perhaps the best approach I can take with this situation that I can offer is that I am a breeder and if this were one of my pups in a litter I produced, it would not be going anywhere, nor would I take a deposit on it nor would I sell it at this stage and I would not promise it to anyone at that pont. If there is something wrong with it I would much rather not have something go terribly wrong in the hands of the pet owner who has now fallen dearly in love with it.
My protocol in such a case is that I keep them see how they do have my vets over time check on them and do a wait and see approach. If things seems to be ok getting well over 6 months of age, my Vet would do the spay/neuter, see how the pup does under anesthesia then if all is going well, proceed from there for a pet home. Time, after it is born and living/thriving under its own life system will often tell both my Vet and I more about the possibility of issues or not and particularly undergoing anasthesia for a normal procedure such as spay/neuter/baby teeth removal will often bring out any issues that may be underlying. Also bear in mind not all life threatening issues are something that can be tested for, often it is a wait and see.
