They can cause dental caries and malformation of the bite if you leave them too long. I'd get your baby in, get pre-op bloodwork done and an exam to be sure he's ok for the procedure and have those pulled immediately. They have tiny roots so it's not that painful afterward I wouldn't imagine and the vet will likely give you something to give for pain.
