I hope everything went well this morning, you might want to include a written letter that gives permission for your vet and the breeders vet to discuss your Max's medical condition and to share health records.
There are at times narrative comments a vet makes on the findings of any physical or wellness exam they do. Each vet may have a slightly different medical form set-up but usually all major organ systems, physical outline pictorial of a dog, any ortho testing results from manual examination, measurements taken, heart rate respiration rate etc. Coat health and weight, condition of paws/nails etc etc. .
For example on the affected "eye" - a vet might note a difference visually in eye size or shape, obviously the results of any testing done, the location of the bacterial infection, the severity of the dryness of the eye. A picture might have been take of the eye and sent onto the opthamologist consultant. Notes from the consultation etc.
I hope this note finds you in time, but it is easy enough to do the letter and either fax it directly to your vet and the breeders vet, or scan the letter into your computer and sent it via email to both and to the breeder.
