Gosh, to me this looks very much like pyoderma (staph intermedius infection). I can't believe your vet just didn't do a quick swab of the area and look at it under scope for bacteria...I'd be a little ticked about that. It only takes a minute and can Dx pyoderma so easily. If it's pyoderma, you truly need to get oral antibiotics, like Simplicef...before it gets worse.

Btw, is he itching anywhere and/or chewing/licking his paws? Oftentimes, dogs with allergies will get a bout or 2 of pyoderma during allergy season bc their skin becomes irritated. Have you noticed anything?
