I see this thread is already 5 pages. I admit I have not read all the answers. I can only tell you that puppies only start to develop the muscle that is able to hold back urine at around 16 weeks. Up until then you really need to keep her near the potty pad at all times. When she has gone on the pad reward her and then you should be able to let her have some free time. Limiting the area she is able to roam in at this age is really important. She is just a baby and can't manage potting in the right place is she is too far away. When a pup is to far from the pad it can't always get to the pad in time. So having constant accidents sets up a pattern of peeing where ever they happen to be.
