Hello and welcome to YT. I, fortunately, have not had to deal with this yet. Yorkies are prone to have issues with their teeth and periodontal disease. I am thinking that your vet will be putting your little one under and will scrape the tartar off his teeth. With periodontal disease, your little one may loose a few teeth (the vet will pull any that they think need to come out). Brushing these little one's teeth is very important. If you have not yet, I would start getting your little one used to having your fingers in his mouth and try to start brushing his teeth. If that doesn't work, there are other products you can use. I give each of my pups a Dentastix at least once a day. I also use an enzymatic toothpaste on Izzy (she is our youngest and the toothpaste I got from the vet - it is CET). I use Tropiclean Fresh Breath Gel on Jack - his teeth are much worse than Izzy's and this stuff works great!

There are other products that you can use and I am hoping others will chime in so they can give you some additional advice.
