It is really very simple, cut off the bottom of the onsie and then take up the excess from the underside (imagine the pins are your seam) then hem the bottom and for the ruffle, I just made a simple pleated skirt and added it to the bottom.
I purchased newborn size onsies because they didn't have preemie. That is why I had to take so much off the underneath part (or back of the onsie)
