Sentinel is Novartis too. Your vet probably ran out of it by now... Sentinel is Interceptor and Program. The Program just stops fleas from reproducing. It doesn't kill adults. You can get Program separately, but that is again Novartis, so I'm not sure anybody has any in stock.
I can only say I'd personally choose Heartgard Plus and then pick the flea med. Yes, only when there is suspicion of fleas. I don't really like flea meds and prefer not to give them constantly.
