While I understand your concerns about him contracting something, it does sound like he needs to see a vet. Perhaps an anti inflammatory will help him?
I would just be sure his little feet don't touch the floor at the vet ... and be sure the vet cleans the exam table. Take a little blanket with you and put it down for him to stand on when the vet examines him.
