I would strongly suggest that you go to a board certified orthopedic surgeon. You will not find one for less than around $1,500. Some vets do the surgery and charge less, but you run a risk of having to do a repeat and it will end up costing more. Best to go to the best from the start in my opinion.

The risk with not repairing a luxating patella is a torn cruciate ligament and then you are talking about more invasive surgery.
