I think you have to weigh the risks and pain of the little guy continuing to live with rotten teeth and all the complications that can cause vs. the risk of anesthesia in an older, tiny dog. Usually, your vet is the best one to advise but you might get a second opinion as to the advisability. My Jilly had shoulder surgery when she was 12 1/2 and weighed 3 lbs. and she came through the anesthesia just fine.
