I am trying to get her to eat the plain NF but I can't force her to consume it. Isn't it better she eats something rather than nothing? Isn't metabolized and excreted by the kidneys- Won't it build up with decreased kidneys functions? I guess I should into this too. What is a common dose for a 5 pound yorkie? I wonder why my vet did not prescribe this since she has a decreased appetite?

Look.....I am giving you the best advice I can. You really need to trust vets and stop reading so much and trying to diagnose and treat this yourself. If you don't trust the vet you have, then go to the Univ. of PA.
I have had more than one pup with chronic renal issues. It all boils down to lab numbers and your vet SHOULD know how to treat renal failure.
Sounds like you are driving yourself and your pup crazy. Seriously. You cannot be forcing water on her...she will drink enough I am so sure. And she gets it in her food, too. IF she gets into numbers that require fluids I am quite sure the vet will recommend it.
As for the Mirtazapine, I speak from experience. Yes, it is an anti depressant. It has been used for many years on dogs and cats. It helps the nausea and stimulates the appetite. As for how it is metabolized, I personally think you are thinking too hard on this. It works and I have used it on renal challenged pups...and it HELPED them immensely. Don't just go read about it...ask the vet. If you don't want to use it ...hey that is your call. It was merely a recommendation because I have used it and it saved my pup's life when he would not eat....I also know others who have used it.
You really seem to be trying to treat this pup yourself and while I think we all need to be educated consumers, we cannot take the place of a licensed veterinarian.
