I hold the treat in my hand with the thumb and pointer finger and the rest of the fingers out and say down and place my hand on the floor. You are right about the eye to floor distance not being too far, so I've also tried it with the dog up on a chair or stool, and putting your hand lower than the chair level. However doing this usually ends up with the dog putting its butt in the air, so with the other hand you need to hover it over the butt, so if it goes up they feel something there and usually not raise the butt any higher.
Another method I have seen Victoria Stillwell use: Sit on the floor with one leg out and bent at the knee to make a sort of tunnel. Have the dog come in from the outside and put your hand with the treat on the inside of your leg. Say down and slowly pull the treat through underneath your leg. If they want the treat they will have to get down under your leg. As soon as the elbows hit the ground, treat them. I have had 50/50 success with this method.
