A vet needs to see what is going on. He does need to wear an e-collar or a sock/shoe if not already so he can't lick the neosporin. So many things can cause redness and itchiness. Mites gather between toes, yeast, it's a spot where allergy itchies happens, they can get lick granulomas there, something may have bit him and irritated things, did he step on something and hurt himself somehow/could it be infected, etc., etc. He needs to see the vet.
