You have to crop the photo into a perfect square first; otherwise you're forcing it into a shape it doesn't want to go.
It depends what software you're using, but often if you hold down the shift key while using the crop tool, it crops in a square. In iphoto, there's a popup box in the lower left corner that allows you to select "square."

Oh, an option is to not crop it in a perfect square. Set the widest dimension at 75 pixels and let the other dimension fall at whatever it wants to be, like 75 x 60 or whatever.
