YES, totally realistic! I've been through many of them, having retired from 35 years in Health Care. We would solicit some of the High School Students as participants. They yelled and thrashed around, pretending to be in pain form various injuries, and the make up was quite convincing.
We would cringe, seeing that some of the patients occasionally were the Football Players... HA, those guys weigh a TON !
