I am so glad Teddy is okay and didn't get away or worse! This is one of my biggest nightmares.
Was it the metal clip at the end that failed or some other part of the leash? I use a regular leash, but the clip that attaches to the harness looks exactly the same. Once when I picked up Max, I must have knocked the little thing that pulls the bar back and the leash disconnected from his harness. Luckily I noticed when I was putting him back on the ground.
