The way we learned was to hold a treat in our fingers and put in front of the dog and motion to the floor. The dog will more than likely follow the treat. It kinda helps if the dog is in a sit position.
Hold the treat until the dog is all the way down and then say down and treat. the next thing after lay is crawl. You should have a treat that can be nibbled--as the dog is in the down position, slowly pull the treat away and the dog will more than likely follow it crawling. If he tries to stand up...just gently put your hand on his back to keep him lowered. Once he crawls, even it's just one paw in front, say "crawl" and treat.
