Some dogs have allergies to oatmeal based shampoos, which are generally touted to calm itchy skin. I think coconut oil based shampoos and conditioners are better for normal and agitated skin.
As you've been dealing with these problems for a time now, it would be wise to go to a Dermatologist straight away. I think they would be better equipped to get to the root of her problem quickly, and get whatever is affecting her under control.
