It sounds like your vet did everything right. There are a couple possibilities. He could have another infection or the drug didn't clear it the first time. This would be itchy and uncomfortable. He could also be irritated in there (may have scratched it open and/or cleaning everyday has caused an issue). Whatever the case, he needs to see the vet again. Hopefully they can get it cleared up. For a few dogs it's a chronic problem though (and allergies can just make it worse).
