She might be smelling her potty scent on the carpet and that's why she continues-even after you provided a new spot and training for it and if you used cleaners on them...have you steam cleaned them after them being removed for a bit? I went through this too.
Slowly increasing her area might help, from crating at night, to a play yard area, to access to only one room...small steps and consistency.
Scoobers used to potty where he'd slept it was "unique" to his prior abuse as it is unnatural for dogs to do this...so we had a hell of a time with potty training...he was scared of grass at first so we use a litter box in the bathroom and wow it was so hard to get there...picking up potty and putting it in the box...steaming the floor-he was only allowed in tiled rooms-washing his blankets and his bed from his play yard-he couldn't be crated it was too traumatic for him to it was like a cage to him. He now has just started using a crate made for a dog three times his size....
We finally got to the point he went in the bathroom in the litter box, then on the pads, and he was okay with grass and outdoors etc...but man was it weird to see a dog come in from the yard and run to the bathroom to potty and then back outside...
Finally we moved the whole thing outside...he had accidents on the bathroom floor and even a regression to trying to find a new spot in the house to potty...so we had to schedule feedings...walk walk walk and reward him every time he went outdoors-I also made a special spot for him to go outside-it was so frustrating the first couple weeks I swear one day I was at my wits end...we walked for over two hours I knew he had to go...finally I gave up we came home and he pooped on the floor immediately upon entering the house!
But we kept on going...and we haven't had an accident in a year now...we got a puppy who needed potty training and Scoobers regressed a little then, but it was mild we worked on it and we stay working on it...he may never be able to be like other dogs and be considered trained he may always have times of regression; as long as he has his schedule of feeding, potty and exercise, and has access to a designated potty area, we see no accidents in the house. But it took a lot to get here!
