Have you had him since he was a pup or did you get him when he was older? the most obvious thing for his nervous ways are if you got him older and he had a nasty owner in the past, but im sure you have already thought of that.
Have you tried switching vets? I know there is a female vet at my surgery who Poppy is really scared of, but a male vet she loves and is totally relaxed with so he is the only vet she sees now.
It's strange for your pup to be so nervous, I would get a second opinion from another vet to make sure he isnt ill.
