Jezebel had the same things done at 7 months -- spay, 2 teeth, and microchip, but she was bigger than your girl, and I was still a nervous wreck!
1. If she is going into heat, I would probably wait, but your vet does seem experienced in handling spay during heat so it is up to you re how comfortable you are with that. It does make things a little more complicated.
2. Sounds like you have plenty of time off. She should be back to "mostly OK" where she can be left alone by the day you are planning on returning to work. She will probably be OK sooner than that.
3. They will shave her tummy.
4. I used onesies, just cut a hole in the backside so that her privates were exposed for peeing/pooping, but the stitches were still covered. At her size, a preemie would probably fit best. Walmart has the Gerber brand in a 5-pack. They worked beautifully. Not only did they keep her from getting to the stitches, but they also prevented the incision from touch the floor when she laid down on it, and prevented things like grass from touching the incision when she went outside, something an e-collar can't do.
