I just picked up a four month old puppy tonight that jumped or fell off of a seat in a car and tore his ACL. His owner could not afford the surgery that was estimated at $2,000 so she surrendered him. This is SO common that yorkies are surrendered due to injuries from falls. That is IF they survive. As you said, a fall can be fatal. People really do not realize how fragile these pupsters are.
