It's hard to get a picture with the info you gave. Can i ask a few questions?
What are you feeding him and how much, how often?
Have you seen Mom diapering him or seen him poop?
Are you burping him after feedings?
Is he feeding from Mom at all?
How long has his stomach been swollen?
It could be gas, he could be full or constipated. Though I've never used it, some here have recommended Mylicon, an over the counter gas relief product for babies. You could try diapering him by using a warm damp cloth on his rear to see if he'll poop. The fluids the vet gave him wouldn't cause this.
I looked up the dosage for Mylicon for pups and I found 1/4 cc per pound so for such a small pup a couple drops should be enough.
